There are tattoo parlours in the Zones. Not all are properly clean, though some are, and you can get tattoos without infections (as demonstrated by Blackout Crypt and Midnight Marauder, who both have tattoos).
The Zones are extremely queer. There still are straights, but it's much more diverse than just that. If you are in any way homophobic or transphobic, sexist (or any of the others), you have the desert after you.
