Aemie's POV

'Acute Osteomyelitis'

Ang Osteomyelitis kasi ay usually na di diagnosed using complete blood count or CBC which shows an increase of White BLood Cells (WBC) as well as elevated Erythrocyte Sedimentation rate (ESR) and C-reactive protein or (CRP) which both specific markers of inflammation.

"Dra. Roswell. Nandito po ang mother nung nasa room 312." Hays. I can see the sadness in her eyes. Hala! Naiiyak ako. Bakit ganoon? Hinarap ko siya tsaka ako nagpaliwanag.

"Ma'am kayo po ba ang ang mama ni Liana?" I asked, pertaining to the patient name tsaka ko tinuro ang kwarto na kinaroroonan ng anak niya.

"Oo, ako nga po ano po ba ang sakit ng anak ko? Hindi naman po siya mawawala diba? Hindi ko na po kakayanin! Muntik na po siyang mawala sa akin isang beses dahil sa isang aksidente! Please doctora tulungan niyo po ang anak ko! Nagmamakaawa ako!" Tsaka siya tuluyang umiyak sa harap ko. Aish! T^T

"Ma'am. Huminahon po kayo. I'll give you a brief review sa kalagayan ng anak niyo. Please follow me." She was just walking beside me. Gusto ko din umiyak! Hindi biro ang pinagdadaanan ng anak niya. But I need to at least inform her without giving her a glimpse of my emotion since sa akin din siya humuhugot ng lakas.

"Base so po sa MRI ng anak niyo. She's diagnosed as an Osteomyelitis patient. Para po mas maintindihan niyo. Ang 'Osteo' meaning bone, o iyong buto po. Ang 'myeo' naman po ay tumutukoy sa bone marrow at ang 'litis' po ay inflammation. So ang Osteomyelitis po ay tinatawag na Inflammation of the bone or bone marrow na nagreresulta ng infection. Your daughter is diagnosed as having Acute Osteomyelitis." Paliwanag ko.

"Hindi po ba malala? Operahan niyo po ang anak ko! Please po! Para lang mabuhay siya. I can sacrifice everything!" Huwaaaaa! Naiiyak na din talaga ako.

"The case is obviously severe ma'am pero compared to Chronic Osteomyelitis mas magaan ang Acute Osteomyelitis. Sa kaso po ng Osteomyelitis, microorganisms such as bacteria reach the bone to cause an infection. Based on our observation ma'am, we find out na mahina po ang immune system ng anak niyo at nagreresulta iyon ng madaling pagkapit ng bacteria sa katawan niya. The bacteria reached the bone through the blood stream called 'Hematogenous Spread'. Gets niyo po ba?" Pagtatanong ko sa kaniya and she nodded.

"Opo. Naiintindihan ko." Kaya nagpatuloy ako sa discussion.

"Dahil po diyan sa Hemotegenous Spread, maaari pong makarating ang bacteria into specific places depends mostly on the age of the person. Knowing the recent accident happened on your child. That's probably the reason why she was experiencing this now since she experienced a trauma which was an open fracture back then and she continuesly exposed into an outside environment that leads a direct contact on the bacteria. Now that the bacteria reached the bone, it will proliferate and alert nearby immune cells such as dendritic cells and macrophages which was obviously fighting that bacteria. Okay po ba? Wala ka pong tanong?"

"Wala po." Ngumiti naman ako. That's the only thing I can offer for now.

"That's result into Acute phase of the disease that lasts into weeks. Pero may mga cases po na nag le lead into Chronic Osteomyelitis na la lasts po into months or years." Bigla na naman siyang humagulhol. Hala! T^T

"But that's not the case ma'am. Hindi po natin papaabutin diyan." I assured her. Hays.

"Please po doctora! Please po!" Huminga ako ng malalim tsaka tumango.

"Ang pinaka most common po na klase ng bacteria ay ang tinatawag na 'Staphylococcus' wherein it lives on the skin and can invade the skin and spread contiguesly to the bone na kung saan 'yon po ang nasa anak niyo. We diagnosed her as a patient of Acute Osteomyelitis since she was showing some symptoms related to it such as pain on the site of infection and fever." I explained it to her one by one para mas maintindihan niya kasi sobrang nalulungkot din ako.

"Ano po ang pwedeng treatment? Does she needs a surgery? Please doctora! Help my daughter!" Bahagya ko namang tinapik ang likod niya.

"So, the treatment we can use po is the weeks of antibiotics directed to the organ. If there's an abscess po specifically in the verterbral absess then surgery is needed ma'am. Are we clear na po?" Tsaka ako ngumiti. Her child is strong, she'll definitely overcome that infection.

"Opo Doctora! Maraming salamat po! Please take care of my daughter. Please po!" Hinawakan niya naman ang kamay ko. Bahagya ko iyong pinisil telling her that it will be alright. We will take good care of her daughter. Obviously, it is my job but I also care for the patient. Tinuturing ko lahat ng pasyente ko as my own family at wala akong ibang hinahangad kundi mapabuti sila.

I smiled.
