An 'Oh shit' moment is not always the same as the 'I fucked up' moment. An example of an 'I fucked up' moment is when I accidentally burned a car filled with explosives when I should've burned the car filled with incriminating evidence of our operations.
An example of an 'Oh shit' moment is when your two obsessive exes meet with your current fling.
