My Hero Academia Lemon one-shots [Requests open]

Intro and Requests

Hello, welcome to my MHA book for lemons. I want to say that I will only do certain people that I will list below the rules and the form that people will have to fill out ONLY ON THIS PAGE. 

Rules for this book:

1-NO X READERS! I refuse to do them cause they are hard to do. I've already tried on a different site to do x readers and that was hellish for me as an experiment. 

2-You must fill the entire form out properly, if you don't your comment will be ignored then deleted if you don't see your error yourself. 

3-Do not ask me about anyone that isn't on the list-I refuse to do a person that I am uncomfortable with doing.

4-Expect a week if not more for your request to be done, I am a busy person with many books-my attention won't just be on this book. 

5-No more than four requests per person-I can't handle that much when it comes to taking requests hence the previous rule.

6-Threesomes are allow...just please be careful with whom you chose >< 

7-Villain sides of heroes and hero sides of villains are accepted

Age: [between the ages of 16 and 26 is what I will work with]

Crush: [Who you want your oc to be with]

Quirk: [What makes your oc unique, I rather not have to deal with an oc that doesn't have one cause that would be rather difficult.]

Friend or acquaintance: [This is rather important, I need to know if your oc is friends with your crush or not for the story board of the one-shot that I will be making for you.]

Personality: [This should be rather obvious, what does your character act like-keep it short and sweet please]

Looks: [Height, weight, chest size, body size hair color and length, eye color-also what do they wear]

Student or pro hero/Villain: [I need to know where they stand in the MHA world, cause depends on where they stand depends on what I do for your oneshot]
