I think the plot twist should be that Niall has a twin and Cindy is unaware that the guy she has two kids with is not really who she thinks he is, and once he finds out that she knows he does something absolutely psychotic 

I think you should make it so louis finds a girl for him that isn't Cindy. And Niall and Cindy really do get married and have a great life! But, it's your book so do it however you want!
