Divide and Conquer, that is the Machiavellian philosophy of war, and that is exactly what antifascist militants did.
 
With the different far-right group at each other's throats, it took only one additional year for them to crumble completely and for half of the country to fall to the left-leaning militants. Six other months were all that was needed for the other half to fall as well. With the fascists beaten, only the legitimist government that had taken refuge in overseas territories such as Hawaii, Puerto Rico and American Samoa remained.
 
With the mainland secured the conquered territories were reorganized into communes with each local community choosing the policies that were better for itself. An overarching organization for the protection of the communes in case of war was established but otherwise, the policies, even that regarding other communes was left for the locals to decide, except of course in territories with high levels of fascist sympathizers, which were ruled by a joint military government in order to de-radicalize them.
 
Seeing this as their chance, the legitimist government ordered a full-scale attack to reclaim the mainland. Much to their dismay however the soldiers simply...refused. Eager to go back home and disillusioned by the increasingly despotic legitimist government loyalist soldiers soon found themselves highly outnumbered by defectors and separatists. And so, Puerto Rico and the Virgin Islands got their independence, Hawaii restored its monarchy and united with Guam and the Mariana Islands, as for Samoa it was reunited with their western brethren into one truly united Samoa.
As for the loyalists, they soon found themselves round up and taken to the Washington D.C commune by the defectors, soon to be judged and imprisoned there. While the equipment left on the newly independent nations were confiscated by local authorities.
 
And so ends the Pax Americana, the long decades of American colonialism came to an end and the various anarchist communes, whether anarcho-communist, anarcho-capitalist, minarchists, anarcho-syndicalists and other types of decentralized communes came to be in its place. Peace came back to the continent as the Cartels were cleared out from Mexico and all conflict seemed to be over...For now.
 
By the end of this war, the political landscape of the world had changed significantly. Norway, Bosnia, Montenegro, Macedonia, Albania and Ukraine ascended to the European Union, which had evolved to also include a military pact in order to supplant the now defunct North Atlantic Treaty Organisation, the EU found itself enlarged and bolstered, in order to avoid complete breakup, the United Kingdom decided to join it back as well, declaring the previous referendum unconstitutional.

The other commonwealth countries entered into an era of increased collaboration and at the end of the civil war, and the EU was already passing motions to have a referendum on economic union. Romania, now more or less ridden of corruption and bolstered by Moldovan European sentiments and Ukrainian friendliness decided to unify with the former, and with Ukrainian support put an end to the Transnistrian rebellion, much to the dismay of Russia.
 
The region was partitioned with referendums in each region over who to join, Romania, or Ukraine. As for the ever-growing protests for Romanian in the newly bolstered democracy of Ukraine, they were also solved with referendums in each constituent, with small parts of Southern Bessarabia and Northern Bucovina deciding to join the Romanian state as well.

The Kosovo dispute was solved by splitting the Northern, Serbian majority regions and giving it to Serbia, and the Southern Albanian majority part and giving to Albania. Thus an era of peace started in that block, peace, if not for the tensions with Russia over the Donbass and Crimea issues.
