Dean doesn't think that Sam is Sam anymore, he thinks that he's something else or someone else, like Lucifer. The boys are on a case that involves people telling the truth and getting other people killed, like a girl killing herself from suicide and a dentist killing a patient.
Later, Liz calls Dean and he says, "Hey." Liz says, "So.... I saw you called." Dean says, "Yeah, it's been crazy." Liz says, "Winter doesn't want to heal the bruise you left on her. I mean, I guess I can't blame her." Dean says, "Liz, I'm sorry, but this is actually the.... worst time in the universe to talk. C-Can we do this later?" Liz says, "You shoved our kid, Dean. How about we do this now?" Dean says, "It wasn't like that." Liz says, "Then how was it? Because even though you do hunts, I still like to know what's going on." Dean sighs and says, "I-I got turned by a vamp. I wasn't really me." Liz sighs and says, "So, then why couldn't you tell me the truth, when you were turned?" Dean says, "I couldn't." Liz voice breaks when she says, "You want to know the truth?" Dean says, "Probably not." Liz says, "You've got so much buried in there, and you push it down, and you push it down. Do you honestly think that you can go through life like that and not freak out? Just, what, drink half a fifth a night and you're good? The minute that Sam walked through that door, I knew. It was over. That our life wasn't gonna be normal anymore. As long as Sam's in your life you're never gonna be happy. That came out so much harsher than I meant." Dean says, "It's not your fault." Liz says, "I'm not saying don't be close to Sam. I mean, I was always jealous of the bond between you two. I've always wanted a sibling who watches my back like how you are with Sam." Dean says, "Okay, Liz. I'm not gonna lie. Okay, me and Sam, we... we've got issues, you know that. But you and the kids--" Liz says, "Me and the kids are fine, it's us. Dean, you're keeping me out me the loop, and I get you want to protect us, but if you keep doing that, you're gonna lose us." Dean says, "I understand and I'm sorry for not telling you the truth. You're right, I keep you out the loop to protect you and I shouldn't keep doing that. We're hunters, it's our life. Our family isn't normal. I guess I just miss the way it used to be. The three of us are hunting together." Liz sighs and says, "Yeah, me too, but Henry needs one of his parents, so Winter brought something up and how about you take her hunting with you? I mean you guys haven't really hunted together before." Dean says, "Sounds like a good idea." Liz softly smiles through the phone and says, "Okay. I'll tell Winter that she can go." Dean softly says, "Are we okay?" Liz sighs and softly says, "Yeah." Dean says, "Okay." Liz then hangs up the phone and Dean has a feeling that he and Liz aren't really okay. Later, Sam and Dean find out Veritas, the Goddess of Truth, is the one doing the killings. They find out the news lady is the Goddess of Truth. The Goddess of Truth tells Sam he's not human and then he kills her. Sam tells Dean he can't feel anything and that he needs help.
