With all the forces running amok in MHA, LOV and the PLF being 2 prominent ones, shouldn't there be more than just Japan dealing with these shenanigans?

With the traitor being him, does this mean that whenever a quirk is given to someone, the body will never be compatible with the given quirk?
If that were the case, in OFA's flashback, a person was given a quirk that allowed their bones to become spikes. How could that body be compatible but Izuku and the traitors not?
If the quirks for Izuku and the traitor are special cases, then why didn't AFO just give the traitor a quirk that wouldn't conflict with the body?
Shouldn't there be a visage of AFO in OFA because AFO passed the passed the stockpile to his brother in the first place?
Does receiving the quirk only hurt if the person doesn't want the quirk? Cause we see AFO's brother having a painful time, but bone spike dude (Who really looks like weird bald dude from ep 1) has no trouble.
What makes the difference between high end and low end nomu's before nomu-fication? Is there a special criteria in the target that allows them to be a high end, or is it just luck?
