Billie Eilish adjusted her oversized hoodie as she waited for her twin sister Rae to finish lacing up her sneakers. It was one of their rare days off, and they’d decided to visit the mall together for a bit of normalcy. Rae, Billie’s twin sister, shared more than her appearance; they both had Tourette syndrome. However, Rae’s tics were more pronounced, compounded by her autism. She often wore ear defenders to cope with sensory overload.
“Ready?” Billie asked, glancing over.
“Ready,” Rae replied, bouncing slightly on her feet—a stim that helped her regulate her anxiety.
As they stepped into the bustling mall, Rae’s hands flitted to her ears before she realized her ear defenders weren’t there. She froze mid-step.
“Rae, what’s wrong?” Billie asked, concern flashing across her face.
“My… my ear defenders. I left them at home,” Rae stammered, her voice already trembling.
Billie frowned. “It’s okay, we can grab some bubble tea and leave. Just a quick trip, yeah?”
Rae nodded hesitantly, though her fingers had already begun twitching. Her vocal tics started up too—a loud “Bop!” followed by a drawn-out “Wheee!” People in the mall turned to look, but Billie stayed calm, gently guiding Rae toward their destination.

The mall was louder than usual—shoppers laughing, children crying, and music blaring from the stores. It was a sensory nightmare. Rae’s breathing grew shallow, and her tics became more frequent. She flung her arm out, nearly hitting a mannequin, then shouted, “Chicken nuggets!” followed by a growl.
Billie, who had her own tics—a sharp neck jerk and the occasional soft whistle—felt her sister’s escalating distress triggering her own. She inhaled deeply, suppressing her urge to hum, but it didn’t work.
“Bop! Wheee!” Rae shouted, her head snapping to the side repeatedly.
“Pfft!” Billie whistled unintentionally, her shoulders twitching. She tried to stay focused on Rae, knowing they were spiraling together.
Suddenly, Rae dropped to her knees, clutching her head. She let out a guttural scream, her face contorted with stress.
“Too loud! Too loud!” Rae cried, slamming her fists against her thighs.
Billie crouched down, shielding her sister as best as she could. “It’s okay, Rae. I’m here. I’m here,” she whispered.
But the commotion had drawn attention. Paparazzi who had been loitering in the mall rushed over, cameras flashing. Rae’s tics worsened under the bright lights and shouting voices.
“Stop! No!” Rae screamed, clawing at her arms as her tics sent her body jerking violently. “Frogs! Frogs everywhere!”
Billie’s heart pounded. She knew she had to act fast. “Back off!” she yelled at the paparazzi, her voice breaking as she tried to suppress her own tics. But they didn’t listen.
Without hesitation, Billie scooped Rae into her arms, ignoring the fact that her sister was taller and heavier. Rae flailed, her tics causing her legs to kick, but Billie held on tightly. She pushed through the crowd, glaring at anyone who got in her way.
“Get out of the way!” Billie shouted, her voice cracking as a tic escaped—a sharp, birdlike chirp.
