This is a Japanese demon [yo-kai].
The tenjo kudari, also known as the tenjo sagari, is a yokai (Japanese demon or ghost) which takes the appearance of an ugly, naked woman with a long tongue and long, disheveled hair.
The tenjo kudari spends most of its time in hiding, living in the narrow crawlspace between the ceiling and the roof.
Every so often, in the middle of the night, it crawls out from the ceiling, upside-down, to scare people.
