1- Jamie Waylett (Vincent Crabbe) got arrested because he was carrying cocaine. That is why Crabbe is different in the last 2 movies.
2- David Holmes (Harry Potter Stunt Double in The OoP) hit his neck to one of the walls. This caused him to be paralyzed.
