Here's the thing with Sirius's name: (this happens before Sirius went to a Hogwarts when he was okay with his family)

Imagine Sirius smirking at his little brother because Walburga just screamed at him then says suddenly "are you serious?!" and then Regulus would be like "no I'm Regulus" and in the background you can hear Sirius laughing his head off. They both ended up being punished but Sirius was so proud of his little brother because that was just plain hilarious.

Now imagine even littler Sirius and Regulus. They started joking with Sirius's name and Walburga became so mad and frustrated. She started calling Sirius by his middle name, which was no help because whenever she called Sirius by his middle name, both Sirius and her husband would answer at the same time. Walburga is enraged by this time.

She's talking with her husband (actually complaining) about their children and she said, "Orion is just... insufferable!" Her husband was very offended, and the question just split out of her with sarcasm, "are you serious?!" like a rhetorical question but her husband looked at his 2 sons in the eye and answered with a grin, "no, I'm Orion."

While Walburga's jaw was dropped on the floor, the other three Blacks were laughing hysterically.

All was good until Sirius went to Hogwarts and got sorted into Gryffindor and befriended all the people his family and friends don't like.

During Christmas break of Sirius's 6th year at Hogwarts:

"Sirius! You get down here right now and apologize to your father!" Walburga Black shouted at her eldest son, who was currently upstairs in his room, packing everything he needed/wanted into his trunk. Sirius was getting the hell out of this hell hole called Number 12 Grimmauld Place.

Dragging his trunk down the stairs, Sirius pushed past his mother without giving her a second glance, a look so blunt and full of hatred on his face he looked like he was about ready to murder someone. He made his way to the door but didn't get there in time because the next thing he knew, he was being dragged by the collar by his das and over to face his mother and to be able to look at her face-to-face.

After a moment, Walburga slapped Sirius across the face, but Sirius didn't even flinch. He started to walk back up the stairs to make sure he grabbed anything but again, didn't reach his destination (his room) when he suddenly fell down the stairs and withered on the floor in pain, crying out

"Crucio!" Sirius heard his father shout, and braced himself as he readied himself for the unbearable pain.

The pain continued for about another 5 minutes when Sirius stood up quickly and ran to his trunk, tears flowing down his chest. With a cry, he dodged the red light that had been shot out to him from his mother's wand.

"Where to you think you're going mister?!" His father hissed, staying where he was standing, his wand still pointed at Sirius.

"Away from this hell hole." Sirius said confidently, grabbing his trunk and opening the door.

"Just face it, you have no where else to go!" His mother said in a shrilly voice.

"Says who! I've got the Potters! I've got the Lupins. Hell, I've even got the McKinnons! Anywhere is better than this wretched place!" Sirius shouted. "I did my waiting! 5 years of it! I have waited for this moment."

His parents stayed quiet. Both thoughts ran in their heads: what must be done, must be done. Walburga looked at Orion who gave a solemn nod. She made it seem like she was about to shoot another spell at Sirius, but she aimed her wand at Sirius's face on the family tree a the last second, and burnt it. Sirius gasped, his eyes and mouth widening. Tears welled up in his eyes.

"What must be done, must be done." Orion said, and Walburga nodded.

"You... are no longer a son of ours." Walburga whispered, her wand pointed at Sirius.

"I'm okay with that." Sirius said strongly and looked around the place he'd once called his home and nodded. He didn't see any sign of Regulus. Sirius turned around one final time and walked out of the house, slamming the door shut behind him.

"What must be done, must be done." Orion and Walburga whispered to each other, lowered and put away their wands, and walked away.

And from around the corner of the staircase, Regulus was watching it all. His role model and big brother, the best brother he could ever ask for, was gone.
