Dean decided that she wanted to be called a he, so she asked John if he would refer to her as a he. Though confused, he agreed and called her by that pronoun. Sammy thought Dean was a he so that's what he always said in his quick way of speaking, like all children his age.
He felt good about it, Dean did. He loved the opposite gender pronoun because it felt like it belonged in the place of she. He finally beginning to feel perfect.
The schools didn't understand, but, Dean decided, they didn't have to. He knew who he was going to be and no one was going to stop him.
