Germany was very concerned he had seen the way that Italy was acting and noticed some changes in his health. At first he thought that it was nothing but then Japan noticed that something had changed with Italy. Slowly had noticed that he was eating less and looked more tired. He noticed other things like how Italy tried to avoid being touched and how he'd flinch when some one grabbed him. He had talked with Japan and booth of them agreed that some thing was wrong , however neither of them know what. Hell, even Romano had come up to Germany asking if he knew what's wrong. That conversation had concerns Germany a lot because he knew that almost no one knew Italy better than Romano. Germany knew something was wrong but he didn't know what. He'll be damed if he didn't try to find out though.
