Yes, those are cuts and a burn mark. They had quite a rough time in the Ruins, as you can see!
Their hair is all scruffy from countless falls and trips and battles, all the Ruins monsters willing to do anything to get their stats up and defend themselves and their loved ones from whatever (or whoever) it is that keeps killing everyone and screwing with the timeline. And yes, they are wearing jeans.
That burn mark is actually from the fight with Toriel. She's much less trusting and not at all hesitant to fight and defend herself now, but she does still love Frisk to the end of the world and wants to protect them the best she can, so she put up the best fight she could, fully risking killing them, in order to either convince them to stay, prove themselves strong enough to defend themselves from the people outside and their enhanced strength, or have them die painlessly in her loving, motherly embrace, since whatever it is that's screwing with time doesn't seem to have a sense of MERCY anymore, and sure likes attacking swiftly and then leaving its victims to suffer to death...
