Name, hair color, eye color, hair length, favorite color, bla bla and what kind of situation you want or if you want it to stay as (Y/n), (h/c), etc. Oh and if you want anonymous or your name/username to be put in.
If you just put my "name is ______ and the rest the same and can you make a One shot", then... Theres gonna be some problems on what I wrote and what you wanted so keep that in mind.
