PLOT TWIST ~ A State of Mind
During the entire book, Hazel was in the hospital. Augustus Waters was purely just a hallucination caused by the lack of oxygen to her brain.
When the doctors were able to provide her body with enough oxygen, Augustus disappeared. Thus causing his death. This explains why her time with Augustus was so perfect, like a dream.
In a way, you could say that Augustus Waters was somewhat of an angel, created by Hazel Grace's mind. She was the one who truly feared oblivion.
