In the story the child known as Zachary is suffering a fatal head injury along with a minor stomach injury, In the ambulance the child regains some small form of consciousness enough to hear his mom but too paralysed to move, eventually it was too late and the child lost consciousness in the ambulance just before he could make it to the hospital, he stopped breathing when he lost consciousness and the doctors/nurses failed to bring him back.
Hence the angel there to take him to heaven.
