Your name. We want to go on a first-name basis with our members, but only if you are comfortable. If not, leave this blank.
Your username. We accept members who use more than one account: one for writing, and one for commenting. However, you will need to report your completion of tasks on each TASK using your main account.
Your pronouns. He, she, or they? You may use others that are not on this list. (Please do not make a mockery of this.)
Your book title.
Your book genre.
Your book language. What language is your book predominantly written in?
Mature? Is your book classified as this? Answer with only "YES" or "NO."
LGBTQIA+? Does your book contain this? Answer with only "YES" or "NO."
Your likes. We are talking about book genres here, not your personal interests. Specify your favorite types of books to read, and I will do my best to assign them to you.
Your dislikes. Unlike the aforementioned section, these are non-negotiable. Any genres or themes specified in this will be implemented to the highest degree. If you are mistakenly assigned a story that violates this genre, please go back to the first bullet under #1 within the MODUS OPERANDI section in the previous chapter to know how to proceed.
Languages you understand. What languages can you comfortably and completely read in? Do not specify if your comprehension is not at a hundred percent.
Your work load. As I said in the previous chapter, you will be grouped according to this. You may choose from the following:
•⠀TYPE A, which will have you do 12 comments on 2 chapters;
•⠀TYPE B, which will have you do 12 comments on 4 chapters;
•⠀TYPE C, which will have you do 9 comments on 2 chapters;
•⠀TYPE D, which will have you do 9 comments on 4 chapters;
•⠀TYPE E, which will have you do 6 comments on 2 chapters;
•⠀TYPE F, which will have you do 6 comments on 4 chapters;
•⠀TYPE G, which will have you do 3 comments on 2 chapters;
•⠀TYPE H, which will have you do 3 comments on 4 chapters;
Your no. of partners. You are allowed as many partners as you want. Remember that your choice here means that you are consenting to exchanging your specified work load for each partner you are assigned. Choose carefully.
Featured book? Answer with only "YES" or "NO."
Your password. To make things spicy, find the first five words in the third sentence of the third rule under RULES from the previous chapter. Do what it says. (Or, more specifically, type what it asks for this section as your password.) Tell me where it came from as well. It doesn't have to be from our club!
Tags. Tag three authors you'd want to join us!

If you are on Web, you can copy paste the form in an in-line comment I left on this sentence.
Any applications and applicants sent in at the beginning or in between a month will be accepted and assigned partners a month after that.
