The Yellow Eyed Demon missed a few of the other Special Children while "All Hell Breaks Loose" was happening, their time to "play" hadn't come around yet. Sam was meant to kill Eva, and then be the reigning champion to guide the next group of Special Children through the game. The demonic part of Sam was also supposed to take him over, like it did Eva, and make him want to kill the others, knowing it would be the only way he would be able to see Dean ever again.
The Special Children that had been missed were all tracked down by a group of demons, and used as the strongest vessels when the huge horde of Demons broke through in preparation for the approaching war.
