The Boys' House 6:00 PM
Peniel, Ilhoon and, Sungjae was still helping Changsub Study while Eunkwang, Minhyuk, was cooking them dinner while Hyunsik went out to buy Tteokbokki as a request by their Inna I-mo. She was really craving for spicy foods that's why Eunkwang and Minhyuk were also preparing Kimchi-Jjigae.
"Explain what is Hypoplastic Left Heart Syndrome." Ilhoon reads the material that Changsub uses to review.
"Hypoplastic Left Heart Syndrome or HLHS is a birth defect that affects normal blood flow through the heart. As the baby develops during pregnancy, the left side of the heart does not form correctly. Hypoplastic left heart syndrome is one type of congenital heart defect. Congenital means present at birth. Because a baby with this defect needs surgery or other procedures soon after birth, HLHS is considered a critical congenital heart defect." Changsub answered.
"What are some of the left structures that HLHS affects that do not develop??" Peniel pressed. Already got himself familiarize with the material earlier.
"The Left Ventricle is affected, it is underdeveloped and too small. The mitral valves is not formed or is too small. The aortic valve is not formed or is very small. The ascending portion of the aorta is underdeveloped or is too small. Often, babies with hypoplastic left heart syndrome also have an atrial septal defect, which is a hole between the left and right upper chambers or atria of the heart." Changsub recited.
"What is the First Surgery done to treat HLHS??" Sungjae questioned as he already knew the answer since he did the said surgery multiple times already.
"The Norwood Surgery. This surgery usually is done within the first 2 weeks of a baby’s life. Surgeons create a “new” aorta and connect it to the right ventricle. They also place a tube from either the aorta or the right ventricle to the vessels supplying the lungs or the pulmonary arteries. Thus, the right ventricle can pump blood to both the lungs and the rest of the body. This can be a very challenging surgery. After this procedure, an infant’s skin still might look bluish because oxygen-rich and oxygen-poor blood still mix in the heart." Changsub's answer made Sungjae nod. Sungjae was about to throw another question but someone beat him to throwing one.
"What is the time limit during Norwood Surgery and why should you not exceed the said limit??" Inna seems to just woke up from her nap. She threw the question after hearing Changsub answer from the room.
"The Surgery should be 45 minutes as the main goal of the surgery is to create a aorta for the baby. Exceeding the limit would kill the baby from the lack of oxygen as we also lowers their temperature to around 18 degrees Celsius to buytime the 45 minutes needed to create the aorta." Changsub smoothly answers.
"Then what is the two next surgeries after the Norwood Surgery??" Dr. Gong sat together with the group Infront of Changsub.
"The second surgery is The Bi-directional Glenn Operation. This operation usually is performed about six months after the Norwood to divert half of the blood to the lungs when circulation through the lungs no longer needs as much pressure from the ventricle. The shunt to the pulmonary arteries is disconnected and the right pulmonary artery is connected directly to the superior vena cava, the vein that brings deoxygenated blood from the upper part of the body to the heart. This sends half of the deoxygenated blood directly to the lungs without going through the ventricle. The third and last surgery is the Fontan Operation. This is the third stage, usually performed about 18 to 36 months after the Glenn. It connects the inferior vena cava, the blood vessel that drains deoxygenated blood from the lower part of the body into the heart, to the pulmonary artery by creating a channel through or just outside the heart to direct blood to the pulmonary artery. At this stage, all deoxygenated blood flows passively through the lungs." Changsub responded.
"You seem ready to take the exam already. That's good. It will give you more than enough time to watch live surgeries and apply it all in practice dummies." Dr. Gong is pleased with Changsub's dedication.
"I know I-mo. That's why I finished reviewing early. I have a lot of videos to watch through before practicing." Changsub replied.
"Well you better get ready as I'm making you the first assistant of majority of the surgeries done to infants. It would be a good way to review and at the same time apply what you read." Dr. Gong informed Changsub.
"Sungjae make sure to help him on some of the CS surgeries." Dr. Gong reminded Sungjae.
"Ne I-mo I already scheduled the surgeries that requires me to perform them. You don't have to think about them too much. It would be bad for you to get stressed I-mo." Now it was Sungjae time to remind Dr. Gong as he stood up to prepare Haetnim's milk so they can eat later.
"Aigoo you guys are all the same. Don't worry about me. I'm tougher than you guys are imagining me to be." Dr. Gong fired back as she hates being treated as such.
Hyunsik arrived shortly after the review session as Haetnim and Dong Wook woke up from their nap already.
Everyone took their seats including Haetnim who was being fed by Inna from her seat. Everyone was enjoying the meal as well as Inna as her cravings was satisfied. The atmosphere of the whole dinner was good along with the conversations.
The couple went home at around 11:00 pm right after Inna tucked in Haetnim to sleep inside Sungjae's room as they're still worried about her despite the baby monitor already installed on the room. The Boys' went to bed right after after walking the older couple out of the house with the mission success in mind as Dr. Gong looked visibly better than the past few days.
As the couple was driving away from the house, Inna can't help but smile at the Boys' protectiveness towards her and her baby.
"Our baby is already loved by them." Inna smiled as she caress her growing stomach.
"We're blessed to have all of them." Dong Wook replied as he peaked through the retreating figure of the guys before focusing his eyes back on the road.

Korea Special Forces Base 2300
The RV squad just landed at the base. The hideout that Kim Won Hyung's group used didn't give them more info so they're pretty bummed out about it. They already left the scene but some investigators remained there while the girls went back to base to dig more intels on the Vice President. All of them doesn't expect to get much sleep but Joy did smile upon reading Sungjae's texts earlier together with an attached picture of Haetnim with her new clothes courtesy of her new Grandmother.
(A/N: So I had a unexpectedly long vacant yesterday that allowed me to write majority of this chapter and also research about the topics for this chapter. Still when's the next one yet but I'll just publish this chapter already. It's a bit boring but whatever I've been out for a long time 😂.)
