WHEN SAM WAKES, HE FIRST NOTICES THE PAIN IN HIS BACK. Sharp. He sits up slowly and takes in his surroundings. He's on a dirty mattress in a familiar run-down house. He's still in Cold Oak.

There's a mirror on the opposite wall. He manages to stand and, despite the pain, he lifts his shirt. He sees the wound in his back, large and sore. It doesn't make sense.

He hears a door open and close, and recognizes Dean's heavy footsteps. "Sammy?" He stops in the doorway, only for a second before pulling Sam into a hug.

He groans in pain, but attempts to return the hug.

Dean pulls away. "Sorry, sorry. I'm just happy to see you up and around."

"Dean– why didn't you tell me?" His breath is partially labored, from the pain and from how fast his thoughts are spinning.

Dean's eyes widen ever so slightly. "What?"

"The demon, he– he told me, he showed me– how could you not tell me?" His breathing picks up more. He brushes past Dean, like he's going to go find her right this second.

"Sam–" Dean grabs his shoulder and moves in front of him, blocking his path. "Just calm down for a second. What the hell are you talking about?"

"That we–" He takes a quick but deep breath. "We have a sister?" He blinks away the burning in his eyes.

"What?" Dean seems just as confused as he felt before. "What are you– Sam, I think we would know if we had a sister."

Sam shakes his head. "No, Dean, it's true. He showed me."

"The demon? The demon told you we have a long-lost sister? And it never once occurred to you that he might be lying?"

He shakes his head again. "The night mom died, he took her. The demon took her. She's– we're twins. I think."

Dean grabs his arm gently. "Okay, look, let's sit down. Tell me the rest."

Sam nods, sitting back on the bed. Dean sits in a chair across from him. "He appeared to me in a dream. He told me about her. He said that she was the strongest of all of us, the...psychic children. And now he's trying to find her 'equal.' someone to lead with her."

"Lead a demon army?"

"He showed me the night he took her. We were sharing a crib. He just...took her. Just like that."

"He showed you the night mom died?"

He nods again. "After that, he...showed me her. Her now. Dean, she's...he has to be hurting her. Or doing something to her. He makes her kill. I know she doesn't want to."

"Did you talk to her?"

He shakes his head. "She couldn't see us. He's keeping her in this...house. A house like this, half rotten, but emptier. And...he said he kept her in a basement. Until a year ago." His eyes start to sting again. "She looks like me, y'know. But she's...she doesn't look healthy. Like he doesn't feed her, or something."

Dean runs a hand down his face. "Shit. And you...believe him?" He says it not questioning, but confirming.

"I know it's true, Dean. I can feel it."

"Like a psychic thing?"

He shrugs defeatedly. "I don't know. More like a gut feeling." He shakes his head once more, this time unable to blink away the tears. "Why would dad keep this from us? Why wouldn't he– we could've been doing something. We could've been helping her."

"I don't know. Son of a bitch kept a lot of things from us. Maybe Bobby knows something."

Sam keeps his eyes glued to the floor, even with the tears now steadily streaming down his face. "We have to find her. We need to get her away from him. Now."

"What's her name? Do you know it?"

Sam drags a hand across his face to wipe the liquid. "He called her Bryce. God...the way he talked about her...like he owned her. Like she's a fucking pet, or something."

"Okay, we'll head to Bobby's and see what he knows. For now, you need to chill out for a second. Are you hungry?"

"I can't– no."

"You need to eat something, Sammy."

Sam stands from the bed, shaking his head. He can barely feel the pain in his back. "We can't wait. Not with this. Bobby's a few hours away. You can tell me what the hell happened to me on the way."

BRYCE CAN'T PULL HER THOUGHTS AWAY FROM SAM. Even when she finds Jake in the woods, even when Azazel instructs her on what to tell him. After she tells Jake where to go, she doesn't go back to Azazel. Instead, she looks for the man who held Sam as he died. Her heart aches at his anguish, and for that, she decides to find him.

She's not really sure why.

When she does find him, he's still in Cold Oak, in one of the abandoned houses. But she doesn't find just him, no, she finds Sam, too. Alive.

Azazel won't be happy.

She watches, listening to their conversation. They discuss a sister, one they didn't know about. A twin to Sam. Something twists in her gut at the sight of him beginning to cry.

The other man– Dean– asks if Sam knows her name. She freezes when he says hers.

He doesn't know anything. He must be mistaken.

The men decide they're going to see someone named Bobby. It must be the older man Dean was with.

She leaves.
