Tiny change, I had to change how long Brenna/Sarah was away from Varath to make her pregnancy make sense. So basically she was free of him for one month, and is one month pregnant.

This is not what I had planned. Not what I had wanted to happen. But it did. The monster found me. Made me use my only hidden card to pull. It has ruined everything. The march through the woods and the long drive back into the horrid city of towers, buildings, and mansions, was pure torture. The Alpha would not let go of me, and I nearly made myself sick with all the crying, the wailing, the sobbing I had to do in order to make my tale true.
He can never know. Never know where they are. I must protect them. I must. I fell asleep the moment we had traveled halfway through the city. Now, I wake to a whitewashed and sterile clinical room. A familiar room. One with charts and screens on the walls. My body covered in nothing but a thin paper hospital gown. Feet bare. Skin clean. Hair brushed and braided away from my face.
The signs do not bowed well for me. I scour my neck, shoulders, arms, and wrists. Finding in all, a small bandage on my neck, bandages on my wrists and shoulders. Placed directly where my veins are easily accessed. The Alpha drugged me it seems, and I didn't even notice. That is a problem. My blood has been taken. No doubt for tests of the fetus. The fetus!
I put my hand through the top of the gown and go to feel my stomach. It seems only recently they had done an ultrasound. The goop still slightly wet on my abdomen. Either they left immediately before I woke up, or the dosage was not enough to keep me asleep, and they have not finished with their tests. Their tests. I nearly gag. The vile taste of acid crawling up my throat. The mere thought of what other things they've done while examining my body taking root in my brain. Examining to ascertain that I am truly, woefully, pregnant. My fears force me towards the metal bin in the corner, rushing up from the bed, stumbling towards the bin, and crouching down, only to feel a stabbing pain and soreness emanating from my vagina.
In that moment everything I've had in the last twenty four hours comes up.

Varath"I hate having to continue with more tests Alpha," Calise mutters to me, his eyes looking around nervously as him and walk down the pristine white hallway. A female nurse following behind with the creaking medical cart, her head bent down, following exactly five steps behind her superior pack members. "But it is necessary that we cover all parameters that what our Luna has claimed is one hundred percent true."
"Of course Calise," I answer numbly, following the older wolf into the examine room where my mate, Brenna, sits quietly on the exam bed. Her head lowered, hands claps upon her thighs, bare feet dangling against the metal bed legs. "I trust your judgment."
Brenna's head picks up at the sound of words. Eyeing all three of us, as Calise, myself, and the nurse come into the room. The nurse quickly setting up the medical cart and monitors next to my mate. Brenna allows the nurse to hook her up to take her pulse and blood pressure. All while Calise lists off the results of his tests to me.
"The results concluded that our Luna is indeed pregnant Alpha," he begins, fluttering through a series of pages, data, and charts. "She is healthy, all vitals and hormonal levels are normal for a properly developing gestational period of four weeks. From the ultrasound the Luna is carrying twins and the embryos have effectively attached themselves into the uterine wall. Here is a list of vitamins, supplements, and extra medicines if needed, as well as new diet plan for the Luna to begin immediately to ensure healthy pups."
