Sam wakes up in the Impala with panic etched on his features

Sam wakes up in the Impala with panic etched on his features. He glances to his right and sees his dad passed out. Shifting his gaze to the backseat, he spots Arabella and Dean in the same dire situation. The demon-possessed truck driver steps out of his vehicle and approaches where they lie. With supernatural strength, the demon rips the driver's side door off its hinges.

Sam's hands tremble as he points the Colt revolver at the demon, tears stinging his eyes. "Back. Or I'll kill you, I swear to God."

The demon smirks, its malevolence evident. "You won't. You're saving that bullet for someone else."

Sam's resolve hardens. He cocks the gun. "You wanna bet?"

The demon's smile fades, and then it vanishes in a swirling black cloud of smoke, exiting through the man's mouth. The truck driver collapses, and Sam uncocks the gun, relief washing over him.

Suddenly, the man screams. "Oh my God!"

Sam looks over to his dad. "Dad?"

"Did I do this?" the truck driver asks.

Sam panics. "Dad! Arabella? Dean? Dean!!"

A rescue helicopter descends to the site, and all four are loaded onto stretchers. John, Dean, and Arabella remain unconscious, while Sam is awake. He cries in fear for his family, panic rising within him. They are taken to a hospital where Dean still lies unconscious in a hospital bed.

Arabella wakes up from her unconscious state and surveys her hospital room.

"What the hell happened?" she asks herself.

Sam enters the room, a soft smile spreading across his face as he sees her awake. "Hey. How're you feeling?"

Arabella smiles back and groans a little. "I'm sore, but I'll be okay. Where's your dad? And Dean?"

Sam frowns. "My dad is awake, but Dean... he hasn't woken up yet. The doctors don't know if he will."

Arabella stands up from her bed and places her hand on Sam's arm. "Dean is strong. He'll wake up."

Sam smiles hopefully. In the corner of his eye, he notices a few unique tattoos on Arabella's arms. For now, he keeps his mouth shut about them.

"I'm going to talk to my father. I brought a change of clothes for you so you don't have to be in a hospital gown all day." Sam hands Arabella a pair of pants and a shirt.

Arabella smiles softly. "Thank you."

Sam leaves the room, leaving Arabella alone to change. As soon as she's dressed, she heads to Dean's room. When she finds him, she gasps at the state he's in—tubes sticking into his body everywhere.

She walks over to him and rests her hand gently on his. "I'm so sorry this has happened to you, Dean. If you can hear me, please hold on. Sam needs you. I know we've only known each other for a day, but I can already tell that you're a fighter and that you'll do anything for your brother."
