Context:
Japan and his family the Minatozaki clan, are the only family in the world to have animalistic features which is the kitsune form aka their original form.
The Minatozaki clan barely had any male members which is why it's considered rare in the family. And Japan (Manjiro) is one of them.
The reason Japan chose to keep his human features is because its due to mostly associated with his mother J.E (Hina).
Once in a while when Japan returns to his kitsune features form, China and S.Korea would often make fun of his ears which is why he's embarassed about it.
