Here's the form. Since they're all short stories, there's no limit like "five stories from each category, however I do ask for genre, just to get an idea of what's being submitted and keep everything organized.

After every contest is closed I delete all the comments. If you commented three books on contest one you'll have to recomment them, sorry about that, but this is going to get very confusing if I do it another way.

The tags are for three (or more) users who may also like this contest. Spread the word!
