Okay! This annoys me so I'm going to go ahead and throw this out here. But its Form, not Forum. Let me set the definitions so you can tell why, and yes, I began saying Form, then switched to Forum, and then switched back to Form again.
Forum: a place, meeting, or medium where ideas and views on a particular issue can be exchanged.
Form: the visible shape or configuration of something.
