Ethan and Casey did confess their love for each-other. But they can't deny the risk of having a romantic entaglement between them. If somehow the board gets to know about their relationship which is not appreciated at all in the medical fields,might cause serious trouble for both of them.
First of all,Casey is still just a second-year resident. She already had to go through a lot in her intern year for Mrs. Martinez's death. More truly, for that Landry. Dr. Olsen. Ethan will never want her to go through the same depressing phase of hearing. He'll be damned if her career gets affected for him. He knows how deadly the hospital gossip is.
On the other side,Ethan has a great deal of reputation,not just in US,but around the globe. If the news of Ethan Ramsey being with a second year resident gets out somehow, it'll cause a major conspiracy. Ethan has so many enemies due to his fame, brilliance and his....decent temper which he forgets to control sometimes. Casey doesn't want to create any dents in his reputation. If people start saying that she only got her place in the diagnostics team 'cause she slept with her boss.... she'll break down for sure.
