This is the sudden flashback that had caused Ciel to act impulsively and made him shoot Kelvin, which ultimately led to his death. From these panels of his flashback, we can see Ciel in two different situations. One who's feet are barely seen, one sitting in a cage and back to the one who is lying on the sacrificial table. The Ciel that we know has not been stabbed because he has no scar or sign that there had been a wound on his stomach. Some say that Sebastian can remove all injury but not even Sebastian has removed the branding on Ciel's back.
So the one who is laying on the sacrificial table is not our Ciel, but rather someone else who shares the same appearance; the real Ciel Phantomhive.
