♔ name ,, - rushingwave - it's a regular warrior's name, the reason behind this name is because rushingwave is constantly rushing around and he lives in riverclan.

Okay. I like the name, it's not too common, and it makes sense. When does this take place, as in, is it alongside canon or not? And if so, when? Pretty sure cats know what waves are, so that's good.

♔ nickname ,, - rush - for his constant rush to get somewhere.

Hmmm, okay. Hopefully this will be in the personality.

♔ gender, sexuality ,, - cisgender male; gay.

Alright. Has he met any cat that he fancies?

♔ clan ,, - riverclan

Same thing as I said in the name.

♔ age ,, - he is currently seventeen moons. he will live to be eighteen.

Oop, as in eighteen moons, or eighteen years...

♔ fam ,, - his family are all alive-

stormywind, brother

mousefoot, mother

hailclaw, father

I wish you'd added in descriptions or something, maybe ages, stuff like that. Sort of common names, but I suppose that's okay. Definitely tie into the water-themed category of names, but it's kind of cool to have themed names (eg. flowers, animals, elements, etc.)

♔ appearance ,, - rushingwave has a sleek, shiny, and short fur. although when poofed up he looks like a giant, usually this isn't the case. rushingwave is on the smaller scale of cats, easily able to fit in small spaces. his fur is a dark grey-blue, and he has big, amber eyes. he has large, pricked up ears that are way too big for his face, and his tail is the only part of him that is constantly fluffy.

This makes sense for a RiverClan cat. The sentences/grammar/capitalization are not the best throughout the forms, but it's easy enough to read I suppose. Alright, good detail with his ears and fluffy fur/tail. Not too little or too much added, but it's enough to get a feel for the character.

♔ personality ,, - rushingwave, hence his name, is constantly doing something. he is a hard worker, doing anything and everything in his power to help everyone and everything. he stars in my book, missing mission, in which he goes on a quest to save his clan from an unknown enemy. rushingwave has claustraphobia, so he loves being near windclan territory, and is great at hunting land prey. he is decisive, but when something goes wrong, he will likely never forget it. rushingwave also tends to make small things appear huge, including how he feels about his family. [aka, they're not exactly on speaking terms.]

Okay, I can see how the name would make you assume that he always is doing something. As we get into the second sentence, I can see a little Gary Sue coming out, because like, I get there are people/cats who are not /that/ selfish, but 100% unselfish, that's borderline "OP." What I'm saying is, that should come with drawbacks, like, he has self-destructive habits, or he's not a good hunter/warrior because he doesn't spend enough time hunting/eating.

The title of that book is like a tongue twister lmao.

But, while you do say he doesn't have any powers/not in a prophecy, going on a quest is similar to that. But why was he chosen? Is he right for the role?

Minor spelling mistakes throughout your bio, I'd go through a proofread those :)!

In my opinion, I would definitely elaborate more on his phobia. Like, you shouldn't just say, oh yeah he has a fear of small spaces, and then just leave it at that. How does it affect him? What made him get that phobia? What do others think of him? Things like that. I think it's very cool (like, not that the phobia is cool, but the making your character more ~diverse~) that you added the phobia in, but you should definitely make sure you talk more about that.
