We can see Theresa breathing heavily and was holding Oath of Judah as she had just finished fighting the Honkai Beast that was summoned there, to which the Honkai beast that came out has the face of Buddha and has a worm like body with a lot of hands, which takes Theresa some time to defeat which put her into this situation as she managed to kill the beast by slamming Oath of Judah in the beast's face, which was it's life source

As the beast fades away slowly she got a call from Mei, with Mei telling the situation she was in a few hours ago and taking care of the situation which involves one of Mr. Watcher's group that she managed to capture one of Mr. Watcher's group and was currently being treated back St. Freya, with Theresa thanking Mei for the information and then left as her mission is done, and headed back to Japan to interrogate the capture member

A Few Hours later

In the infirmary 

We can see Sakawa wearing a hospital gown(with those who treated her shocked to see that she had small horns in her head) patting Diablo in the head and was waiting for her interrogator to come inside as one of her hands were cuffed, to which she can just break easily but chose not to since she had already done her job

And then someone entered the infirmary revealing Theresa

Sakawa: So your the Principal...Did you have a decease that causes you not to grow and as you get older you try to hide your old face through surgery or something???
