Susie Campbell awakens aching all over near the dangling cages, and notices her claw like hands and her new black body, confused and scared. She remembers her desire to become Alice, so she gets herself up to find Joey, unaware of her surroundings. She makes her way down a corridor and encounters a bunch of ink monsters, but shows she's not afraid using her claw hands to slash them into bits. Soon, she found herself a little lost in a part of the studio she hasn't seen before, but finds an axe lying around and takes it for protection. She finds a mirror and rushes to check her appearance. But as soon as she looked, she wished he hadn't. With one look seeing her deformity as an imperfect Alice for the first time, she remembers what Joey said and promised to her. Realising that promise was broken, she punches the mirror in a sudden rage, cracking it. She doesn't know why she did that. For a second she felt as if something was taking over her brain and urging her to let it out. It's as if another mind was trying to take over her body. She slowly breaks down in black tears, devastated now that her heart is broken and is now something she had not hoped to be. Drying the tears, Susie got a hold of herself and decides to find Joey to fix this.
As she went on, she finds a coffin. With curiosity, a little bit of concern, she removes the lit only to stare is shock. Much to her horror losing the grip of the lid, it was Susie's own corpse! Terrified, she knew she had to find Joey now. She finds herself next to the elevator system and makes her way to the toy workshop. There was also a statue where screams and whispers could be heard. Meanwhile, Norman Pulk emerges from the darkness with his new body and his light vision, letting out that roar of his heard by some Lost Ones. Soon enough, he gains the name the Projectionist. He could only see what was in his light, but he could not hear. He began to wonder around without a purpose, just like many other monsters.
Susie sees Joey making his way and secretly follows him. He meets with Tom on a balcony near the machine and asks him what he did with the "reject", as in Susie's drenched soul. Tom explains he took her to a place where she was safe from harm, and as well as taking her body to a deathbed like all the others. As they talked, Susie listened with caution. She realises how she was tricked with charm and gave up her own body to be one with Alice, only to become what she is now. Stuck on the thought her promise was broken, Susie starts chopping the Bendy cut outs scattered all over the place in a result of anger. But she soon realizes she shouldn't have done so. The demon, who doesn't like it, arrives and gives chase. He catches her and she ends up in the "screaming well of voices". Tortured, she gets out of the same statue and backs away from it, vowing to never go in there again. She hears the demon again and hides in a nearby miracle station, hoping she won't be seen, despite not knowing she can use it with every monster. He walks past as if she wasn't there.
Coming out of the station, she takes another look at her appearance. She could see rejection in her own reflection, and felt her dreams grow cold. What does she do now? She felt something crawling over her, something telling her she must do something. She wanted to be Alice for real, so is she just going to stay like this? Noises interrupt her thoughts and goes to see what was out there. She discovers a functioning Butcher heart, with the need to take it. This signals the Projectionist and he chases her to the elevator. During the voyage, Susie becomes fixated on what to with this heart. She ends up gobbling it, making her feel the opposite of coming apart. She felt so much better, but a little disgusted. In fact, she felt like her broken parts were mending. She thought if she really wants to be perfect, there are risks she must take.
As time went by, she began to harvest the origins of the Boris clones and Butchers to become the character she desires, while humming Lonely Angel, and is watched by some horrified Lost Ones who flee to the Harbor. To do so, she set up traps and made herself a little domain. She goes to fetch her corpse and finding the tommy gun that belonged to her late father, and builds herself a domain. She is given the name Twisted Alice, because of her twisted ways that are unlike the real one with the goal of perfection. She soon sets that trap for Buddy, fascinated by "the most perfect Boris she had ever seen". She poses as "Milla" by making false messages and leading him to his doom. However, she failed to catch her prey and he escaped. Sometime after that, she hears familiar voices and goes to check it out. It was Allison and Tom, now husband and wife. Susie watches as they are drenched too, and expresses hatred towards that the "copycat" supposedly stealing something she loved.
A while later, Twisted Alice was near the studio's entrance ripping a Boris' heart out and writes the ink message "who's laughing now", when she hears the door opening. She retreats back to her domain, not without hearing Henry's voice. A little later, she hears some talking and takes a look to see Henry and Buddy making their way. And that's when she got an idea. She watches Henry enter the Toy Workshop as she hummed Lonely Angel, and jumpscares him soon after. She encourages him to follow her to the elevator, while taking a bunch of Alice dolls and cut outs, and takes him to her domain of Level 9. But she gets interrupted by a Piper who crawled in trailing his ink. She slashes it and tapes it to a machine to shock it dead, when Henry enters. She tells Henry she will help him out of here, calling him her "errand boy", if he does a few small favours first. Every time she finished or began and errand, she would speak random dialogues coming from her twisted mind, both human and monster. But all the while, she was still determined to get her hands on Daniel when she had the chance. When Henry encountered the Projectionist, he shot him with the tommy gun leaving him to lie weak for a while. Once it was over, Henry was ready to go home. But, just as she had planned, Twisted Alice hijacks the elevator, crashing it. She drags Buddy away, going passed the Lost Ones who were frightened of her, and straps him to a table, wandering what to do with his body after consuming his heart.
The Projectionist is healed from the gun wound and wonders passed the Accounting Department all the way down to Bendyland. He spots Henry near the train and chases him to a miracle station, only to be beheaded by The Ink Demon. Twisted Alice was still spying Henry, bribing him to come after her. In the process, she took the hearts of a Piper and Striker too. In the haunted house, she knew just what to do with Buddy. She takes all the items Henry collected and begins to remodel hm, making him into Brute Boris. When Henry enters the haunted house, she sends Brute Boris to kill him, only to get caught in a brawl herself with some Searchers. Henry defeats Brute Boris, shocking an enraging Twisted Alice. In a rage, she comes running slashing her claws and grabs Henry by the neck to scratch his face, only to be killed by Allison from behind. Twisted Alice ends up in the well of voices again, and covers her ears in agony until she escapes through the bendy statues. Recovering from it, she remembers what happened, and shouts in anger as a result.
