Jack Fain emerges from the ink as a Swollen Searcher near his desk and roams the sewers, gaining the nickname Swollen Jack. He soon discovers one of the 6 items, an Inkwell, and collects it, assuming it might be useful later on, and a few collectable figurines of some familiar faces, even one of himself (at this point, audio logs, food, the 6 items and old cartoon reels are now collectable in each level).

Taking place during the events of DCTL, Sammy Lawrence, now a masked psychopath, was placing Bendy cut outs all over the place to appease his "saviour". Joey approaches him, telling him that if he sacrifices some workers it might please The Ink Demon. He does so by taking Jacob, Richie, Dave, Norman and eventually Buddy too, but misses Dot. Right after Norman, Dave and Jacob, were finished, it was time to take care of Buddy and Richie, though he didn't realise Jacob was still alive. However, in the Music Department, he is knocked out by the projector by Dot and is left stranded.

He gets to his feet and complains how his victims got away, when Jack and some other Searchers pop up. But Sammy wasn't afraid and commands them to stand down, which they do. Thinking he can use them to help set themselves free from the inky prison, he recruits and leads the Searchers and Miner Searchers, and the Lost Ones who soon join them, all the way down to the soon-to-be Lost Harbor, using some Ink Makers in the process. He starts to write the ink messages such "he will set us free", "it's time to believe", "the sheep will come to slaughter" and "down here we're all sinners" all over the studio and soon develops the ability to walk through walls. Some of the Lost Ones begin building the harbor underground and did the same thing, writing messages such as "the creator lied to us", "I don't want to work here anymore", "what am I?" and "I still remember my name". Meanwhile, Jack just wonders around the sewer near the boiler becuase he likes the quiet. Sammy still appeased Bendy with blessings, but still needed the right sacrifice.

A while later, he was listening to a radio while writing "he will set us free" on a piece of paper, when he heard someone enter. He turns the music off and hides in the walls, seeing it was Henry. As he watched the Ink demon attempting to grab henry, Sammy had a hunch he would be the perfect sacrifice. He watches Henry approach the Music Department, while asking for an amen and carrying a cut out. He tells some Searchers to go after him, but they are beaten easily. Once he gets Wally's keys, learns Sammy's favourite song to get into his sanctuary and turn the first valve to stop the leak to blocking Sammy's office, Sammy scares Henry moving a cut out and commands more Searchers to attack. But they are beaten too. With no other options, he takes Wally's dustpan from the closet and knocks Henry unconscious to prepare him for the sacrifice. He sings his little sheep sheep song, but much to his dismay Ink Bendy goes for him instead, sending him to the well of voices.

He comes out totally outraged by the "betrayal" and makes a long roam back to the Lost Harbor, uttering to himself. Once Henry enters the Harbor, Sammy crawls through a vent and attacks him in a rage. He is bested, however, and gets his mask knocked off. He grabs Henry and prepares to cut his head off. But before he could, Tom sneaks up from behind and kills him, sending him to the well again.
