"I can't believe we're back here." Fiona said looking around the place with Lily and Lip sitting beside her waiting to be called for them to take Ian back home. "I know. It makes me want to throw up." Lily said playing with her ring not liking of being back here.

"Are you still married?" Lip asked looking Fiona as she looked down at her ring. "I think so. I'm not sure. Gus is silent." Fiona explained with a small laugh, "and you know how good I do with silence." Lily and Lip nodded knowing what she means.

"Silence in our house usually means someone stopped breathing." Lily said as the three of them laughed. "Exactly." Fiona agreed looking down at her ring examining it. "Anyway, he's going on tour for a while, so... I don't know. Maybe it's a good thing." Fiona said as she was still trying to figure out her marriage.

"You'll figure it out." Lily said grabbing her hand making her smile. "I hope so." Fiona said before pointing down at Lily's ring. "What about you? You figured it out yet?" She asked her and Lily shook her head.

"We both live still live at the house full of kids, what do you think?" Lily said making her laugh. "You're right." Fiona said laughing at her sister. Lily then looked at Lip who seemed to be in deep thought. "Why so quiet? You figure out tuition?" Lily asked nudging her brother's knee.

"No." He answered, "no, I got an idea, though."

"Is it an idea that could land you in jail?" Fiona asked seeing him smirk. A woman then walked up to the three siblings. "You're here for Ian Gallagher?" The woman asked and the three nodded getting up from their seat. "Come with me." She said leading them into a room.

"I thought Mickey would be here." Lip said and Fiona rolled her eyes. "Yeah? I didn't." Fiona said as they followed the woman. "Oh, give him a break." Lily said defending the Milkovich boy.

The three of them were now sitting in an office letting the woman explain Ian's diagnosis. Lily and Fiona have all heard it before and they know how to deal with it because of their mother. "Bipolar one, acute mania with psychotic features. This means–" The woman tried to explain.

"No, we know what it means. Can we take him home?" Fiona interrupted her, Lily kicked her leg as it was rude of her to abruptly interrupt her.

"I understand that there's a family history, and that this diagnosis is difficult to hear, but I do need to go over a few specifics as regards to Ian. He's resisting his diagnosis. We had to make med-compliance a requirement for his release." She explained and Lily nodded understandingly.

"All right, so, you're saying we need to hold him down and make him take his meds?" Lily asked and she nodded. "Well, I'm saying that it will take his meds about two weeks to really start working. Until then, yes, I am suggesting he be carefully supervised." She said and Lily nodded fully understanding the situation.

"I'm giving you a one-week supply of antipsychotics, mood stabilizers and sedatives. He needs to take one pill from each bottle, with food, three times a day. You have a week to get him to a psychiatrist if he's insured and–"

"Or a clinic if he's not, which he's not. We know the drill." Fiona added once again interrupting.

"Don't be surprised if he mostly sleeps until his meds are properly balanced." The woman said and Lily gave her a smile. "Ian should be here shortly." She said before leaving.

They waited for a minute before seeing Ian walk in with a blank expression. "Hey Ian." Lily greeted smiling at her brother pulling him into a hug with Fiona and Lip doing the same.

"Let's take you home."

"Hey, look who's home." Lily said walking into the house greeting everyone making everyone smile seeing Ian home. "Well, would you look at this. It's like a great big family reunion." Sammi said as Debbie and Liam came greeting their brother.
