I learned this phrase when I was in high school. Out teacher told us that in order to brainstorm you need mental struggle.
Here is my perception about this phrase. Thinking of a story is hard and it requires mental struggle. Some people became crazy because they keep of thinking things to the highest degree. When writing your story, just don't expect that much. Feel it like it is a river that flows swift and sure in one direction. Do not put your mind in the middle of the storm. Whatever comes in, write it. If you don't like it, change it.
Explain the story well and think of a nice scenario but don't think too much that you would lose your mind.
