I made this a song based fanfiction. The song is "The Love Struck Kitty Won't Be Discouraged!". But the ending is a little bit different. Hope you enjoy ;)

There was a cute little kitten in Japan. Her name was (f/n) and she was living happily. However, she had a problem.
She was in love with a boy. A human boy.
She is always looking at her loved one. When he is in school, when he going to school, when he is coming back from school.
And today was an another day which you are looking at your crush, Shiota Nagisa.
You sent your gazes at him, but he doesn't notice you at all!
Look at me Nagisa!... Look at me...
You were always trying your best for him to look at you but he was always thinking about other things and don't notice you at all!
"Nyaa~ Nyaa~!"
Nagisa look at you and your little cat eyes sparkle with happiness. He smile and pat your head.
"Sorry, but I don't have got something to give you..."
"Hurry up Nagisa-kun! We are going to be late!"
"Okay, I am coming!"
He give a glance at you and smile again.
"Goodbye!"
"Nyaa~!"
You are going already?...
Nagisa run after his red-haired friend. You give out a sigh (a cat sigh) and walk to your favourite place, the shrine of the goddess of cats.
You climb the stairs and go through two cat statues.
You stand at the stairs which is in front of the monetary offering box.
You think about Nagisa again, how he smiles and how he pats your head.
I know that it impossible for me to be with me... But I don't want to stay like this until the end...
You look at the shrine and with deeply.
Please Kami-sama! Please hear my wish! I don't if it is a spell or magic! But please let Nagisa to notice me!...
And that was the time when a miracle happened.
A circle appear on the ground, under you and a bright light coat your little body.
Another dazzling light appear in front of you which you couldn't look directly. A little girl appear from the blinding light.
"I will actualize your wish, my dear. But you must remember that if a human being touchs you, you will turn back to your normal form."
"From what?!"
The little girl smile and start to disappear.
"I wish you luck..."
You close your eyes and when you open them again you noticed that something is weird.
"Something is different..."
You close your mouth with your hands.
"Wait!! Did I speak now?! Wait wait wait! I have got hands?!?"
You look at yourself and see that you have got a human body.
"Kami-sama... Thank you!"
You bow at the shrine and run to Nagisa's school.
Now I don't have got tail or ears... I can meet to Nagisa, I can talk to Nagisa!...
You go through the school gates and climb the stairs to Nagisa's school building quickly.
You climb, climb and climb.
You finally stand at the top of the stairs and see the building. You run towards it and when you stop, you open your mouth to shout.
~Meanwhile in the Building~
"And when you add 3x to 4y-"
"Koro-sensei, there is someone outside!"
Maehara shout and everyone's attention turn to the girl outside.
She open her mouth and shout a familiar someone's name.
"NAGISAAAA!!!!"
Now, everyone's attention was on Nagisa. Nagisa was surprised to hear his name and stand up. He head to the window with a shocked face.
She open her mouth again and shout again.
"OHAYOU!!"
You wave your hands like crazy with a really childish smile.
"Wait Nagisa! Are you friends with that bishoujo?! I will kill you!!!"
A dark aura appear from Okajima.
"I don't know her Okajima-kun!"
"She looks like she is as old as you, minna-san. Then how about invite her Nagisa-kun?"
Koro-sensei ask to Nagisa and he look at the unknown girl.
"I don't know who is she but... I don't think she is bad person!"
"Then let's call her!"
~Back to You~
You see that everyone is looking at you.
Did I do something wrong?...
A yellow creature appear in front of you suddenly and you jump with surprise.
"Hello! May I learn your name?"
He is the teacher of Nagisa... But my name? I don't have got a surname though...
You smile at Koro-sensei.
"I am (f/n)! I want to be a student here!"

After that, you be friends with people from 3-E and of course, Nagisa.
You studied like a normal student and when you learned that there is an another language called English, you were too shocked. However, Nagisa help you learning it.
You take part in the race at the sports festival and support the boys when they are doing the pole-toppling.
And when you learned that there is something called Valentine Day, you gave a chocolate to your dear Nagisa.
Your ordinary cat life had turned into an enjoyable life which is filled with love and friendship.
However, there was a problem.

You were going home with Nagisa and who knows why, you two start to talk about hands.
"Karma-kun's hands are prety big too. He has got long fingers."
"Koro-sensei doesn't even have got a hand. Or does he?"
"But they are tentacles..."
"You can't know~."
He giggle and you giggle with him.
I can talk with Nagisa... Even though it is about hands... And, I my cat paws are as big as Nagisa's hands right now...
You raise your hand to the air and look at you with curious eyes.
"What are you doing?"
You raise down your hand and fake a smile.
"N-Nothing!"
He take out his hand from his pocket and smile.
"Want to see whose hand is bigger?"
When you were going to touch his hand, you remember something really important.
"I will actualize your wish, my dear. But you must remember that if a human being touchs you, you will turn back to your normal form."
You slowly put down your hand.
"Sorry..."
"..."
A sad expression appear on Nagisa's face but he just smile sadly.
"Don't worry..."
Nagisa start to walk and you just stand there.
When he was going to walk across the street, you call out his name.
"Wait Nagisa!"
And then, the accident happens.
He just ignore your voice and start walking. Suddenly, a car come from the road and you feel that it is going to hit Nagisa.
"Look out! Nagisa!"
Nagisa turn to you and when he is turning, he see the car.
The car notice Nagisa too but couldn't stop.
And from now on the things happened, happened in slow motion.
I must save him... I must protect him!...
The car was getting more near to Nagisa and then, you jump on him and make him move from his place. You fall down with him and look at him if he is injured.
Then the world turned to it's normal speed.
You look at Nagisa carefully.
"Are you okay Nagisa?"
"Yes, I am okay thanks to you."
He smile and you smile back at him.
"I am glad you are fine Nagisa."
"I am sorry for not waiting you (f/n) and really thank you."
You take his hand.
"It isn't a problem at all Nagisa. It is all good if you are okay!"
His eyes meet with your deep (e/c) eyes and then his eyes slowly slide to top of your head. You look at the end of his gaze and see your cat ears.
Ooopppsss.... I did something wrong...
You stand up and run away.
"Wait (f/n)! Please don't run!"
You couldn't find a mirror so you dash to a river.
You look at the reflection of yourself on the river's surface.
The spell Kami-sama casted on me... It is disappearing...
You continue looking at the river and you notice that you remember this river.
It was the river which you saw Nagisa for the first time and saved you.
~Flashback~
In a rainy day in March, you were in a box and shivering.
Suddenly, you heard some footsteps coming to you and you look up.
There was a boy with blue hair, looking at you with a smile.
"Are you okay neko-chan?"
You look at him with suspicious eyes but he bring out something from his bag. He extend it to you and you see that it is a piece of chicken. You first lick it and eat it up at the end.
The little boy giggle and pat your head.
"Wanna come with me neko-chan?"
He stand up and start to walk. He turn to see if you are coming and you run behind him.
He take you to a little secret base and head in. He put down his bag and take a bandage from somewhere.
He try to grab your paw but you attempt to scratch him because of the sudden action.
Finally, he success to make you calm and wrap it to your little paw.
"Hi neko-chan! I am Nagisa!"
After that, you two sleep in the secret base until the rain stop.
After rain stopped, Nagisa stand up and take his bag. He lean down and pat your head.
"Don't worry neko-chan! I will come back!"
After that day, Nagisa came to the base everyday. He gives you water and food everytime he comes and plays with you. He even gave you a red collar!
One day, Nagisa decided to take you to his home.
He opened his door with a smile and called out his mum. However, his mother wasn't happy like him. He get angry with him and said that it was really hard to take care for an another living being.
At the end, Nagisa couldn't take you as his pet and his mother made him to leave you.
He take you to the place where he found you that rainy day.
He pat your head and fondle under your eye with teary eyes.
"Sorry (f/n)... But it looks like I can't meet with you anymore..."
He stand up and go without saying another thing.
Wait Nagisa... Don't go!!
~End of the Flashback~
You see that Nagisa is coming from your behind from the reflection.
"(f/n)?..."
Isn't it good enough?... I was able to take a human form and talk with Nagisa, eat with Nagisa and smile with Nagisa... Everything good has an end... If this is my end, I am happy enough and I want Nagisa to be happy too... So I must give him the best goodbye then!...
You turn to Nagisa with a smile.
"I love you Nagisa, I really do!"
He blush and you blush with him.
Even if Nagisa forgot me and our special fay... I don't care! I love him and it won't stop!...
Nagisa slowly take your hand and smile.
"I love you too, (f/n)!"
You feel that tears are going to fall down.
"I am really glad that I was able to meet you Nagisa."
The tears start to fall down and you squeeze Nagisa's hand.
"Don't say like that you are going to die (f/n)..."
"But I won't be able to talk to you again..."
You slowly start to sparkle and some dazzling lights from you fly towards sky.
Nagisa hug you and you hug him.
"One day (f/n)... We'll meet again... And I will wait until that day."
And finally, you start to get smaller.
You close your eyes and whisper.
"Mata ne Nagisa... (See you again...)"
Nagisa try to hug you tightly but, you weren't there anymore. However, he was feeling that he is hugging something.
He open his eyes and see that he is hugging a cat. A sleeping cat.
Don't worry (f/n)... I will wait until you open your eyes...
He kiss between your eyes and take you to his home.
~Timeskip~
Nagisa wear his pyjamas and look at you, who is on the bed.
He get inside of his blanket and try to sleep while hugging you.
She is just sleeping so it must be okay right?...
And slowly, the blue-haired boy close his eyes.
~Another Timeskip~
You open your eyes slowly and see that is very dark.
Where am I?... What am I doing?... What are these things wrapping me from behind?...
You turn to your behind and meet with blue cloth.
It is really warm...
When you were going to close your eyes again, you remember what happened.
You try to escape and try to move.
"W-Wha?! What is happening?!"
"Nagisa?!"
Wait did I speak now?!....
Nagisa open the blanket to meet with your (e/c) eyes. With the human from ones.
You look at each other and hug each other at the end.
"Welcome (f/n)!!"
"I am home Nagisa!!"
You nuzzle to Nagisa's neck and whisper with a lovely voice.
"I love you Nagisa..."
"I love you too (f/n)..."
"And I think your pyjamas are really cute!"
"(f/n)!!"

"Tehehe~!"
~One More Timeskip~
You were going to school with Nagisa but you saw a familiar shrine and stop.
"Can you wait here a little bit Nagisa?"
"Okay but be careful!"
"Are you my mummy?!"
You climb the stairs and head to the little offering box.
It was bigger when I was a cat...
You bring out 100¥ and throw it to the box. You clap and bow.
Kami-sama... You can't understand how much I am grateful to you... Thanks Kami-sama!
You smile and climb down the stairs quickly.
"Slow down (f/n)! You are going to fall!!"
"A cat always fall onto 4 legs!"
