Again, there's two different ways this AU can go. Either Stan remembers his adulthood once a child, or he doesn't. (I've also seen fics where Ford is the one that shrinks, but that's not important rn)

In the case of Stan remembering his adulthood and everything that happened, not THAT much changes. Of course he has the handicaps that a small child has (e.g. he can't reach high places) and since Ford is working on a cure most of the time, Bella has to help. She finds it adorable when Stan gets frustrated and thinks that Stan as a kid is super cute, even though he keeps insisting that he's not cute. Bella takes over the roll of being in charge of the Mystery Shack (because Stan flat out refuses to close the shack ever) and Bella, being nervous and shy in front of a bunch of people, keeps making mistakes that make Stan cringe.

The age difference defiantly makes things awkward in their relationship. Bella acts more like a mom than a wife, and while Stan understands why, he still doesn't like it that much.

In the (more common) case that Stan doesn't remember his adulthood, things change quite a bit. Of course he doesn't remember Bella, as they first met after the portal incident, and is really shocked to find out he even has a wife. I've seen some people write that Stan knew and liked Carla as a kid, so if that's the case he at first assumes that Bella married Ford when he hears 'Bella Pines'. (Unless, of course, the author headcanons Ford as gay and that Ford knew and told Stan at whatever age Stan is shrunk to)

When Stan doesn't have his memory, Bella spends more time helping Ford find a cure or keeping to herself than interacting with young Stan. While she does still love him, she feels awkward when he doesn't exactly know who she is, and therefore prefers to just read or write by herself (although she still voices how she thinks Stan is really cute as a kid).
