After you desire from your heart to change and realise only Jesus can help you and that he loves you. You should KNOW what the problem is. Yes Jesus is forgiving but he should see you making that effort to change as well. The Bible says shall we continue in sin that grace may abound? Also, Jesus wants that effort so the Bible says If your right arm causes you to sin cut it off. It is better to enter the kingdom of God with one arm. So that effort is necessary. And before you make the effort you should IDENTIFY what the sin is. Apart from the sin you already know, there could be more you don't know. So the only way to find out is the Bible. The basic commandments in Exodus 20. You can write it down so you don't forget and make a CONCIOUS effort to change. So you Know and not assume. The Bible is a big part of our journey so have it handy, on your phone, laptop or tab. If you prefer the hard copy, go for it. There is usually a big issue on how do I even begin to read this big book. Hold on, everything will be clear soon.

Identify what theses sins are and make CONCIOUS effort to change. Keep praying.
Identify the reasons for sin as well. Cut it off! It could be a person; best friend, partner etc.. An event? Anything it is. Cut it off! And you can't do this on your own of course Jesus is there to help you.
