The main reason for Karkat's insomnia is the fact that he's a mutant.
Every other blood caste has some idea of how long they'll live.
Karkat doesn't.
He's always been afraid that one day he'd just die in his sleep.
The only time he lets himself actually sleep deeply is when he's with Dave.
Because he knows if he dies sleeping in Dave's arms, at least he died happy.
Eventually that's how it happened.
