Head cannon:
When Lance returned to Earth after, what, 6 years? He doesn't become a farmer, no, he becomes a doctor. Why? Because he's seen so many people get hurt and die that he'd rather die again and not see it happen anymore. You cannot tell me this boy wouldn't work hard to get a doctors degree while also hiding it from the other paladins.
When he graduates from college 8 years later he cuts himself off from the paladins. They don't see him until the ambulance gets to them when they're saving people from the random galra that had survived the war.
They barely recognize him because of how much he changed in, like, 4 years. He doesn't even give them a second glance as he rushes to the people and help load them in the ambulances and helping them stop the bleeding(mostly) then he goes back in and kills the galra and leaves the building and says "More helpful than I was as a paladin, huh princess?" while walking back to the ambulances and tending to small wounds on bystanders cause the paladins are reckless asf.
