Madeleine Grey wasn't an regular attending at Seattle Grace hospital, she is the first born of Ellis Grey and Thatcher Grey and she is the older sister to Meredith Grey.
Relationship between Meredith and Madeleine has been rough, they were close growing up but that all changed when the older sister left for medical school and life moved on.

Meredith Grey is an intern and Madeleine is a fellow attending for cardio.

Madeleine had been in Chicago from her first year as an intern to her two month of fellowship before Richard Webber gave her a call.
