It's been seven long strenuous weeks of study hall, going to the pit and writing reports for Dr. Knowles. To make matters worse she would make me stand in front of her and give her reports on all my patients, and then she would critique me on what I could do better and make me do it all over again. I was dreading meeting her in her office right now, but I had something for her. I found a cool cardiac case today; I did a full cardiac workup with the permission of Dr. Lawson of course.

I arrived at Dr. Knowles office door, her office was basically the penthouse of the hospital, located on 8th floor. You never knocked on her door, you rang the buzzer and she'll look at the camera and decide to let you in. Yea she's an annoying boujie bitch with a boujie ass office to match. Her office was the size of a house strictly white, black and gold, overlooking the skyscrapers of Manhattan.

The door unlocked and I walked in, the scent of her intoxicating N°5 welcomed my senses. She sat behind her desk looking like a snack per usual. If I ever experimented in the rainbow, she would definitely be my first pick, her alpha male demeanor definitely attracted female attention. She motioned her hand for me to take a seat, she was typing away at on her laptop with her glasses at the brim of her nose.

"I suppose you have written up your last and final rapport?"

"Yes, I did."

"Let me hear it."

"19-year-old male came into the ER displaying signs of typical congestive heart failure. This morning he woke up extremely short of breath, tried to stand up but fell straight to the ground. He's lost range of motion in both extremities. According to his history you placed a loop recorder 6 months ago and an ICD 3 months ago. But unfortunate-"

"He's getting worst..." She sighed holding her head in her hands. "What's your approach Maraj?"

"The only approach and liable procedure for him right would be an LVAD. But looking at his last echocardiogram done at 1400 hrs. he has an ejection fraction of 27% and his heart is size of a football. The LVAD would not be strong enough to take on the stress his heart will give it. He will definitely need a heart transplant, very soon. I'd give him less than three days to live give or take."

"Hmm..." She said taking of her glasses leaning back in her chair. Her fingers drummed against the table. "Good job Maraj."

My jaw dropped. "HUH? No feedback? No do it over again?"

"You did well, although you missed the fact that I sent him to the ER two days ago. He was the patient that you ruled out to have a long-term congenital heart defect. What made you change your approach this time?"

Now it all made sense. "You sent him?"

"He's my patient...The same patient that was here two days go with an ejection fraction of 41% that is now 27% because of your incompetence! Now back to my question, what made you change your approach?"

I was about to feel sick. "I- ughh I just asked Dr. Lawson to do some cardiac workup on him, because the last ones were over four months old."

"So, you took initiative?"

"I guess but that doesn't justify you using this poor little kid as teaching lesson. He could have died!"

"EXACTLY! Because you didn't take initiative." She got up grabbing her white coat off the coat rack nearby and swung around. "In 10 minutes, James heart will be landing from Mercy. If you wish to scrub in, his procedure begins at exactly 2100."
