John Kramer is the central antagonist in the Saw franchise. He was diagnosed with cancer(but given the wrong time frame of death), and his wife was hit with a door which made her go into labor later she lost the kid, and he tried to off himself but he survived.
He took this as a sign from god that if someone deserves to live they will fight for it. So he kidnaps people and puts them through his experiments to see if people who throw their lives away would fight for it if they knew they would die if they didn't. Amanda, a druggie, was tested by Kramer and she succeeded so he took her in as an apprentice.
Along with his “first” apprentice Dr. James Gordan, the man who diagnosed John Kramer with cancer, was tested and he succeed, his family was also threatened. John Kramer later had another apprentice Detective Hoffman.
This is when Kramer realized he made a mistake taking Hoffman and Amanda in, they didn't want to help people change their lives, only wanting the thrill of watching people suffer and rigged games so people would never survive.
Eventually, multiple people get tested and fail while the survivors either wind up apprentices or go off into the world.
