Here's the overdue explanation of the ending.

Jake turned Jerome into a vampire when he almost died. By the time Mitch found him, he was already turned and somewhat already insane. In this AU, when vampires are created, they have a chance of being possessed by a spirit, as they are half-dead. They act as a bridge between the living and the dead during transformation and if one particular spirit takes the opening... Anyway, Jerome already had a dormant spirit inside him from the time of the transformation, but his mental and physical strength was enough to keep it from doing anything.

As Jake warned, Jerome required more and more blood over time. When the withdrawal happened, Jerome got weak and the spirit took over at several intervals. Eventually, the spirit took over his body, and almost all of his mind, explaining the small spurts of humanity in his thoughts. However, most of the time the real Jerome was somewhat dormant inside his body although he was experiencing everything. Therefore, he remembers everything.

As Jake mentioned briefly, it is possible to take a spirit out of one vampire and into another. This is what starts the buildup to the ending. The reason Adam and Ty were told to keep Mitch away was because he knew Mitch didn't want neither Jake or Jerome to die. Jake concealed the fact that he was basically committing suicide because he didn't exactly think it was a big deal. Jake wasn't part of the group, no matter how close he got to any of them, he didn't think that was something they needed to know. Either way, he needed to make up a quick story that hid the truth about what the real plan was, and Jerome dying seemed to be the first thing that came to mind. As for Adam and Ty, they didn't agree, but couldn't do much about it anyway. After all, Jake was a 2,000 or so year old vampire with superhuman powers. The best they could do was just cooperate, although Mitch convinced them to disobey later.

All the capturing of Jerome, the weaponry, all the preparations were made for a set plan. The plan is somewhat complicated, but the basic idea was that Jake would perform the ritual to take in the spirit and after that, would kill himself before it could readjust and take over him. However, with Jake's personality and the way he maintains himself, the spirit wouldn't have been able to anyway. No, Jake wanted to die, he loved Mitch in a way, but he knew Mitch didn't belong with him. The thought of dying was something Jake had already thought about, due to his experiences and such, but he didn't have a good reason until now.

When the trio finally made it to where the ritual was happening, the spirit was already inside Jake. Jerome was unconscious, therefore he had no recollection of the events until the other three told him. They just happened to come in the exact moment where Jake was about to stab himself, and all three of them got the wrong idea. Neither of them expected Jake to kill himself at all. Nevertheless, Jake's plan succeeded, even with the unexpected intrusion. The ending chapter takes place one week after the funeral, three weeks after the suicide. It's insinuated that Mitch visits the graveyard once a week, maybe more.

Last thing I wanted to explain is Mitch's feelings towards Jake. Mitch did love Jake, he even kissed him at one point. However, no matter what, Jerome was the one who mattered more to him. Jake was more of a friend, a good one at that. Jake to him was a shoulder to cry on, someone he could run to for support, someone he loved like a friend. The kiss was more like a friendly kiss, maybe a little more to both of them, but not enough to act on.
