Russia doesn't wear his coat and scarf to protect himself from the cold (he's used to it by now). He wears them to hide his scars from all the times he's self-harmed. They cover his neck, his chest, his arms, and his legs. He cries himself to sleep at night because he "knows" that the other nations will never understand how much pain he has gone through.
