Step 1:

Open up the image below as the background, and an image or PNG of your choice as the foreground.

Step 2:
Put your image on lighten. If your image isn't a png, mask away the extra background of the image until it looks something like this.

Step 3:
Put the glossy overlay on your cover. Put it on overlay, and then turn the opacity down to about 25.

Add a filter, title, and a quote and then you're done! Here's the final product.
