Pacific Southwest Airlines Flight 182, registered as November 533 Papa Sierra (N533PS), was a scheduled domestic passenger flight from Sacramento International Airport (IATA: SMF, ICAO: KSMF), Sacramento, California to Los Angeles International Airport (IATA: LAX, ICAO: KLAX), Los Angeles, California and then to San Diego International Airport (IATA: SAN, ICAO: KSAN), San Diego, California, on the 25th of September 1978, the Boeing 727-214 serving this route and the flight collided with a private Cessna 172 light aircraft, registered as November 7711 Golf (N7711G), over San Diego, California, the accident was PSA's first fatal accident and the collision and crash remains the deadliest air disaster in California history, all 135 people on board the PSA plane were killed in the crash, the 2 people in the Cessna were also killed and 7 people on the ground were killed and 9 other people on the ground were injured (144 in total were killed).
At 8:59am local time, the PSA crew was alerted by the approach controller about a small Cessna 172 Skyhawk aircraft that was nearby, the PSA crew reported that they saw the Cessna after that the ATC noticed the PSA crew of it's position to the aircraft, shortly after the PSA crew no longer had the Cessna in sight and they were speculating the position of the aircraft, the tower controller received a transmission at 09:00:50am local time from the PSA jet that the Cessna was passing off to the right of the PSA plane and the ATC assumed the PSA jet had the Cessna in sight, thus maintaining visual separation, despite the Captain saying the Cessna was behind the PSA jet, the Cessna was directly in front and below the PSA jet, the jet was descending and was rapidly closing in on the small plane, which took a right turn to the east, deviating from the assigned course, PSA Flight 182 took over the Cessna, the collision occurred at about 2,600 feet, witnesses stated they heard a loud metallic crunching sound, then an explosion, the 727's right wing was burning, the wreckage of the Cessna plummeted to the ground, it's vertical stabiliser tore away from the fuselage and bent to the aircraft, the debris from the Cessna hit around 3,500 feet northwest of where the 727 went down, the PSA's right wing was heavily damaged, rendering the plane uncontrollable, which sent it into s sharp right bank, the fuel tank inside the wing ruptured and started a fire, the PSA jet struck a house, it impacted the driveway of the house at 480 kilometres per hour, nose down while banked 50 degrees to the right, impact with the ground occurred at 09:02:07am local time.
The summary of the crash was a Mid air collision resulting from pilot error on the Boeing 727 and ATC error.
