Titles are pretty hard to pick and covers can be hard to make. Here are a few things to give you ideas.
Book Titles
Pick something meaningful to the story. It doesn't have to be something obvious. I like more subtle titles. You can pick a theme that runs throughout the story. Maybe an item of significance. Something to do with the end goal. Like, if their goal is to acquire the golden chalice, you could name it The Golden Chalice. It could be a significant setting (I have a story named In Shado. Shado is a significant setting). You could also just name it after the main character. This works especially if your character has a notable name like Daniel X by James Patterson.
Chapter Titles
I'm not going to lie, I suck at these. Recently I've given up and started using Roman numerals. Usually, I just pick a random word from the chapter that sticks out a little more than the rest. It could be a notable element in the chapter or some kind of theme or danger. Something like that. Also, if you're going to switch point of views, I would put the character name here after a dash or something rather than adding their name to the actual text.
Covers
Covers vary depending on who's making them. You can picture what is titled (the golden chalice). The main character(s). Maybe an intriguing scene from the story. Pick something that stands out/ something important in the story. And since so many of you ask, no I don't have any suggestions for people who will make you a cover. I make my own. There's a cover design club, so you could just start a thread and they'll find you.
