Flashback: In North Dakota, Sam, Amelia and Ruby manages to catch up to Dean and Mary, get the truth to her disappearance, and urges Mary to find Betty and kill her. Mary refuses, and emphasizing that Amelia is managing her hallucinations and atoning for her past actions. Dean agrees to stop badgering his daughter, but later they track and Mary was forced to kill Betty as she was going to kill her sons, in front of Betty's daughter because she believes that Betty would have killed more humans in the future. Mary doesn't kill the daughter who hasn't killed anyone, but warns that if the daughter kills anyone, she will. The daughter promises that the only person she will kill is Mary. Throughout, the Leviathans are shown to maintain an eye on the various rocker aliases of the brothers, using the aliases to catch up to them. Later at a bar, the Leviathans in disguises as bikers, sat at the bar aside Sam while he got his beer, one of the bikers pour some type of serum into Sam's beer, that made him fall into a coma. Later, Ruby got Amelia to place a tracking spell on her red cloak, that leads her to the Underworld. End of flashback

Present day in the Underworld, Ruby is surprised to find herself in the Underworld, as she had been pursuing the Leviathans with a tracking spell, Ruby believes the wolf in her has destroyed another life, Sam's, and she wants to make it right. Renee explains that some Leviathans is in the Underworld, and suggests they confront her. Hades informs Clay/Leviathan of Ruby's presence, and Clay decides to kill Ruby. Ruby realizes that Sam's late father, John Winchester, might be in the Underworld, and they find him at a diner he runs. He agrees to give a blessing into a bottle for Ruby to bring to Sam, but Clay appears and turns John into liquid, by contaminating his soup with liquid from the River of Lost Souls, and bottles him up. He declares the Storybrooke visitors anathema, and that anyone who works with them would be under penalty of the same punishment by Hades. Ruby is distraught, and Dorothy asks her if she loves Sam, Ruby admits it, but is afraid to try to give Sam true love's kiss, as she doesn't know if Sam ever reciprocated his feelings. Dorothy convinces her to try. Nyx goes to Hades' lair, where he pours John Winchester into the River of Lost Souls, and explains that he has done this only because the Winchester's is the Leviathans' enemy. They share a kiss and they also share a drink. Later, Ursula asks her uncle to give back her status of goddess, but Hades makes a deal with his niece and tells her what she needs to do, in order to get her status back. Meanwhile, Edward discovers that Ursula is having the phone booth removed by Black beard, Ursula says sorry to Edward. Edward and Dorothy become so distraught, Liam insists that one of them must return to Storybrooke to be with their son, Eddie. They assume it must be Edward, at Hades' lair, Hades thanks his niece for removing the phone booth and so he wave his hand over Ursula as she gained her status of goddess back and she felt more powerful again, she was very delightful and she thanks uncle Hades for restoring her status back, Ursula puffs away in Cyan cloud of smoke. Ursula takes the outsiders to the cemetery and she uses her goddess powers to erase all the outsiders names on the tombstones, allowing everyone to leave. Using the Ruby Slippers, Dorothy and Ruby depart for North Dakota. Hours later, Grace conjures her Hat showing that Sam Winchester was successfully revived, and that Dorothy is with Eddie in Storybrooke. At Hades' lair, Clay arrives to get Hades to let him free, but Hades reveals that he has failed him and Hades push Clay into the River Of Fire.
