Its description is:

4156157143", which decode to "noeunitnoc", or "continue on" backwards.
It shows mainly some televisions in the middle of a woods, probably Rosswood Park. The screens show:
-at 00:12 Alex from Entry #3 (Hoodie can be seen between the trees as well);
-at 00:25 two brown eyes, Tim or Alex's eyes, with the words "SEES NOW", meaning that Alex is watching;
-at 00:32 the word "PERMANENCE", it might suggest that Hoodie "existed" from, or even before, Entry #3;
-at 00:34 Jay from Entry #42;
-at 00:35 Tim from Entry #20;
-at 00:36 Jay from Entry #42 again;
-at 00:37 can be seen a reflex of Hoodie.
It contains several frames of Jessica from Entry #32.
The two set of numbers at the end decode to "trulyyourfault" and "shesoutthere".
